On June 1, 2018, a FlySax plane crashed in the Aberdare mountains, killing all 10 people on board. The accident has been attributed to the pilot and first officer's negligence and poor communication.
According to an investigation report released by Transport Cabinet Secretary James Macharia on November 7, 2019, the crew failed to properly prepare for the flight and made a decision to fly despite unfavorable weather conditions.
"The crew failed to properly prepare for the flight and made a decision to fly despite the unfavorable weather condition and the pilot failed to act on a warning sent by the terrain avoidance and warning system alerting them of a potential hazardous terrain situation," the report read.
The flight, operated by East African Safari Air Express, was en route to Nairobi from Kitale when it crashed. The report also noted that the crew failed to act on a warning right before the crash.
Mr. Macharia also pointed out that there was no pre or post-impact fire, and the emergency locator transmitter did not send a signal as the antenna had broken off due to impact.
