This affidavit confirms that they are the child's parents or have taken parental responsibility over the child.
Once the affidavit is in place, the applicants can move to the civil registry and fill out relevant forms (Forms B1, B3, and B4) to petition the registry of births to amend the original birth certificate. The applicants must also pay relevant fees and obtain receipts from the registry.
