Dyspareunia, or painful intimacy, is a condition that can affect women for various reasons, including physical, health-related, and emotional factors.

Pain during or after intimacy, medically termed dyspareunia, can occur and may be felt in the pelvic area or the inner pelvic region.

All women should be aware that experiencing pain during or after intimacy could indicate an underlying issue and should prompt a visit to a healthcare provider as soon as possible.

The following are reasons why some women may experience dyspareunia:

  1. First-time intimacy – It is common for a woman having intimacy for the first time to feel pain due to the stretching of the hymen, which can be very painful.
  2. Dryness, lack of lubrication – When there is insufficient lubrication, penetration can cause sharp pain and potentially lead to tears. Using a lubricant can help alleviate this issue.
  3. Mental block – Intimacy can be painful if the woman is not fully ready or comfortable, emphasizing the need for emotional and physical involvement.
  4. Infections – Certain sexually transmitted infections (STIs) like chlamydia, gonorrhea, and genital herpes can cause intimacy to be painful.
  5. Menopause – As hormone levels change during menopause, it can lead to vaginal dryness (atrophic vaginitis), resulting in painful intimacy. Lubricants can be beneficial in these cases.
  6. Vaginismus – A condition characterized by the tightening of muscles in or around the vagina, making penetration painful or impossible.
  7. Pelvic inflammatory disease (PID) – An infection of the female upper genital tract that can cause painful intimacy.
  8. Endometriosis – A condition where tissue similar to the womb lining grows outside the womb, leading to painful periods, lower abdominal pain, and painful intimacy.
  9. Fibroids – Non-cancerous growths near the womb, vagina, or cervix that can cause painful intimacy.
  10. Irritable bowel syndrome (IBS) – A digestive system condition that can lead to painful intimacy, among other symptoms like stomach cramps and changes in bowel habits.
  11. Constipation – A common condition that can cause painful intimacy due to the hardness and lumpiness of stools.
